Toilet in warehouse leaking from the tank.The connector from the tank to the water valve looks like it needs replacing plus some parts too- per Seth's observations and pictures. Pictures sent in an email.
Notes: 
looked and watched during flushing and filling. Could not find any water leaking. will check again later today to confirm. If it's seen happening please find Jaison or Tom.
